<p><strong>Hazard: </strong>The flashlights can heat up, smoke or melt when turned on, posing fire and burn hazards.</p>
<p><strong>Incidents/Injuries: </strong>BJ’s has received two reports of incidents with the flashlights, including one unit overheating and one unit burning and making a loud noise. One minor injury to a consumer’s hand has been reported.</p>
<p><strong>Description: </strong>The recall involves Superex SAFETO GO LED flashlight and battery sets. The flashlight sets include one 9.5-inch flashlight with 10 LEDs, two 7.5-inch flashlights with seven LEDs and two 6-inch flashlights with five LEDs. The sets also include seven “D” size batteries and four “AA” size batteries. The flashlights in the sets are made of either red or black plastic with black or gray rubber around the handles and light bases. The words “Superex,” “SAFETO GO Family Pack,” and “Bright White LED Flashlights” appear on the front of the packaging. The UPC 0-56986-01008-3 is printed on the back of the packaging along with the SKU number 700935.</p>
<p><strong>Sold at: </strong>BJ’s Wholesale Clubs from July 2011 through December 2011 for about $15.</p>

<p><strong>Remedy: </strong>Consumers should immediately stop using all of the flashlights and return them to any BJ’s Wholesale Club for a full refund. Consumers should properly dispose of the batteries before returning the flashlights to the store.</p>

<p style="text-align: left;"><strong>Incidents/Injuries: </strong>The firm has received two separate reports of a dog chewing and ingesting gel from the gel pack. One dog reportedly died from ingestion of the gel. The other was reported to have received treatment and has recovered.</p>
<p style="text-align: left;"><strong>Description: </strong>The recalled product is a Ci Sport three-piece, expandable, insulated lunch box set, which includes the lunch box, aluminum bottle and the freezer gel pack. The lunch box of the recalled set has the code &#8220;1-61731-99-57&#8243; printed on one of the two white labels that are sewn under the white fastener inside the main compartment. The lunch box is 8 inches wide, 5 inches deep and 10.25 inches high. It is made of vinyl, polyester nylon and crushed nylon and has a logo with the words &#8220;Ci Sport&#8221; attached to the upper left corner. The lunch box has a carrying handle on the top and a mesh carrying pouch for the aluminum bottle on the side. The aluminum bottle measures 2.75 inches in diameter and 7.5 inches tall. The gel pack is a 6-inch by 4-inch transparent plastic pouch filled with blue liquid gel. The words &#8220;Cryofreeze&#8221; and &#8220;Ice Pack/Hot Pack&#8221; are printed in white letters on the front of the pouch. The lunch box was available in four colors: navy blue, red, black and denim blue.</p>

<p><strong>Hazard: </strong>The wiring for the lamp’s light sockets can become exposed, posing a risk of electric shock to consumers. In addition, use of the recommended standard 40 watt light bulbs can generate excessive heat, which can melt the double plastic shades over the bulbs.</p>
<p><strong>Incidents/Injuries: </strong>The firm has received four reports of melting lamp shades. No injuries have been reported.</p>
<p><strong>Description: </strong>This recall involves Classic Quarters Five Light Floor Lamps. The lamps stand about five feet tall and have a gunmetal or chrome colored metal pole and five adjustable lights mounted on flexible metal tubes at the top. Dark plastic or multi-colored double plastic shades cover each of the five lights. Recalled lamps use standard incandescent or CFL bulbs and have labels marked with the model number “G-1843-5” affixed to the underside of the lamp base. These lamps can be further identified by SKU numbers 612007239, 612007829 or 612008982 at the beginning of the instructions. Lamps that use candelabra bulbs are not subject to this recall and can be identified by labels marked with the model number “G-1844-5B” and SKU numbers 612009036 or 612009037 in the instructions.</p>
<p><strong>Sold exclusively at: </strong>Big Lots stores nationwide from April 2010 through November 2011 for between $30 and $50.</p>

<p><strong>Hazard: </strong>When the toasters are first plugged into the outlets, the heating element can be energized although the toaster lifter is in the up or off position, which can pose a fire hazard if the toaster is near flammable items.</p>
<p><strong>Incidents/Injuries: </strong>Hamilton Beach has received 5 reports of toasters being energized when first plugged into an outlet. There have been no reports of injuries or property damage.</p>
<p><strong>Description: </strong>The Hamilton Beach recall involves model 22602 toasters. The model number is printed on the bottom of the toaster. The toaster has a chromed steel exterior, a front control panel with a rotary toast shade selector and function buttons arranged in an arc, a front removable crumb tray and “Hamilton Beach” printed below the control panel.</p>
<p><strong>Sold at: </strong>Mass merchandisers and department, grocery and home center stores nationwide from August 2011 through November 2011 for between $19 and $34. Also, some of these toasters were sent to consumers as replacements for model 22600 toasters recalled in June 2011.</p>

The massagers measure 3 1/2 inches tall and 5 inches wide and were sold with two Kendal brand AA batteries. &#8220;Massage Pets&#8221; and &#8220;Dating code 1210A&#8221; are printed on the massager&#8217;s hangtag along with UPC 0 49696 60453 6 for the ladybug and UPC 0 49696 60454 3 for the bee.</p>

<p><strong>Incidents/Injuries: </strong>Liebherr has received 16 additional reports of doors detaching on the freestanding refrigerators, including two reports of injuries involving bruising and strains.</p>
<p><strong>Description: </strong>This recall involves Liebherr freestanding 30-inch wide, bottom freezer refrigerators with model and index numbers listed below. The refrigerators were sold individually or as side-by-side companion units. The refrigerators come in a stainless steel finish. &#8220;Liebherr&#8221; is on the upper center of the refrigerator door and the top interior control panel. The model number can be found on a label located behind the bottom drawer on the left interior side of the single door refrigerator.</p>

<p><strong>Hazard: </strong>The mirror glass can detach unexpectedly from the wardrobe door, fall and shatter, posing a laceration hazard to consumers.</p>
<p><strong>Incidents/Injuries: </strong>IKEA has received reports of 14 incidents, including one minor injury in Europe. No incidents have been reported in the U.S. or Canada.</p>
<p><strong>Description: </strong>This recall involves a wardrobe door with full-length glass mirror with model number 101-604-51, supplier number 12650 and production weeks 1039 through 1048 (YYWW). The product name &#8220;PAX AURLAND&#8221; and &#8220;IKEA&#8221; and production week are printed on the rear of the mirror door on the lower edge. The mirror door is made of foil-coated particleboard and the mirror of glass. The mirror is secured with metal fastenings at top and bottom. The mirror door measures about 191/2 inches wide, 93 inches high and 7/8 inches thick.</p>
<p><strong>Sold exclusively at: </strong>IKEA stores nationwide from October 2010 through September 2011 for about $60.</p>
<p><strong>Manufactured in: </strong>Sweden</p>
<p><strong>Remedy: </strong>Consumers should immediately contact their local IKEA store for instructions on returning the mirror wardrobe door for a free replacement door.</p>
