<p><strong>Hazard: </strong>When the adapter is used on a stroller that also has a wheeled board accessory attached for transporting a standing toddler, and the car seat is positioned so the child faces forward, the car seat can disconnect from the adapter and fall.</p>
<p><strong>Incidents/Injuries: </strong>Bugaboo received one report of the car seat disconnecting from the adapter and stroller frame, causing a minor injury.</p>
<p><strong>Description: </strong>This recall involves the Bugaboo car seat adapter models 80400GC01 and 80401GC02. The adapters are devices designed to attach car seats to stroller frames. They are made of silver aluminum tubing and black plastic connecting parts.</p>
<p><strong>Sold at: </strong>Babies &#8220;R&#8221; Us, Buy Buy Baby, Neiman Marcus, other department stores and independent juvenile stores, Bugaboo.com and other online retailers nationwide from December 2005 to July 2011 for about $45.</p>

<p><strong>Hazard: </strong>The stroller canopy&#8217;s embroidered logo&#8217;s backing patch can detach, posing a choking hazard to babies and young children.</p>
<p><strong>Incidents/Injuries: </strong>The firm has received six reports of children mouthing the detached patch. Gagging and choking were reported in two incidents. The backing was removed from the children&#8217;s mouth without injury. In each of the reported incidents, the children were seated in an infant car seat attached to the stroller.</p>
<p><strong>Description: </strong>This recall involves all B.O.B. strollers manufactured between November 1998 and November 2010. Strollers manufactured after October 2006 have a white label affixed to the back of the stroller&#8217;s leg with the manufacturing date. Strollers with no manufacturing date listed were produced prior to October 2006 and are included in this recall. The strollers were sold in single seat and double-seat models. The BOB®, Ironman® or Stroller Strides® brand name is embroidered on the canopy of the strollers.</p>
<p><strong>Sold at: </strong>REI, Babies R&#8217; Us and other children&#8217;s product and sporting goods stores nationwide and Amazon.com between November 1998 and October 2011 for between $280 and $600.</p>

<p><strong>Description: </strong>The recalled strollers have metal frames and were sold as single strollers or with a doubles kit to make them double strollers. The Explorer has three wheels and the Hammerhead has four. Both have a cloth seat and canopy. Explorer strollers were sold in the following colors: black, apple green, navy, red/black and apple green/black. Hammerhead strollers were sold in black only. The phil&amp;teds logo is located on the crotch piece of the harness on both models. The strollers have the text “phil&amp;teds model EX explorer” or “phil&amp;teds model HH hammerhead” printed on a sticker on the rear axle bar. The serial numbers run consecutively with the month and year followed by the unit number. Affected serial numbers: 0610/0001 to 0111/4788 (June 2010, unit #1, to January 2011, unit #4788). This information can be found on a sticker attached to the inside of the left hand hinge when viewing the stroller from behind.</p>
<p><strong>Sold by: </strong>Sold online at philandteds.com, pishposhbaby.com, and albeebaby.com; and at Babies R Us and specialty juvenile retailers in the United States and Canada from August 2010 through June 2011 for between $475 and $500.</p>

<p><strong>Remedy: </strong>Consumers should immediately stop using the product and contact phil&amp;teds to arrange to receive a upgraded brake assembly. Explorer owners will receive a new frame fitted with an upgraded brake assembly. Consumers can perform an in-home swap out of the affected frame. Hammerhead owners will receive a pre-paid return shipping container in order to return the stroller to phil&amp;teds where the brake assembly will be replaced and the stroller returned.</p>

<p><strong>Incidents/Injuries: </strong>CPSC and Arm&#8217;s Reach have received 10 reports of infants falling from the raised mattress into the bottom of the sleeper or becoming entrapped between the edge of the mattress and the side of the bed-side sleeper. No injuries have been reported.</p>
<p><strong>Description: </strong>This recall involves a product called a &#8220;co-sleeper&#8221; by the manufacturer. One side of the bed-side sleepers is lower than the others to allow positioning near a bed and access to the infant for care and feeding. This recall includes all Arm&#8217;s Reach Original and Universal styles with manufacture dates between September 1997 and December 2001. The manufacture date and model number can be found on a sticker on one of the product&#8217;s legs. Model numbers included in the recall begin with:</p>

<p><strong>Description: </strong>This recall involves &#8220;Troy the Activity Truck,&#8221; which is blue, red and yellow with plastic star, circle and heart beads attached to the bead runs on the back of the truck and has a face on the front. &#8220;Infantino&#8221; is printed on the front of the toy truck. Model number 153-210, 206-110 or 506-110C is printed underneath the truck. When its switch is turned on, the truck plays music and the back wheels spin.</p>
<p><strong>Sold at: </strong>Babies R Us, Toys R Us, Meijer, TJ Maxx, Marshalls and other toy stores nationwide from September 2009 through February 2011 for about $15.</p>

<p><strong>Hazard: </strong>The battery in the handheld video monitor can overheat and rupture, posing a burn hazard to consumers.</p>
<p><strong>Incidents/Injuries: </strong>Summer Infant has received five reports of ruptured batteries, including three incidents of minor property damage. No injuries were reported.</p>
<p><strong>Description: </strong>The recall involves Summer Infant Slim and Secure handheld color video monitors with unmarked, MP and BK rechargeable batteries. The Video Monitor is sold in either silver and white, model #02800; or pink and white, model #02805. It has receiver and camera components. The receiver is approximately 4 ¼” tall and 2 ½” wide with a 2.5” LCD screen with the “Summer” logo printed in white on the bottom front. The camera is silver and white. Both the video monitor and receiver components come with A/C adapters but only the receiver unit contains a rechargeable battery. The batteries are unmarked or marked with letters MP or BK on the lower right corner of the battery. Batteries that are marked TCL are not included in this recall.</p>
<p><strong>Sold at: </strong>Exclusively at Babies R Us from September 2009 to May 2010 for about $200.</p>
<p><strong>Manufactured in: </strong>China</p>
<p><strong>Remedy: </strong>Consumers should immediately stop using the video baby monitors with the recalled batteries and contact Summer Infant to receive a postage paid envelope to return the defective battery in exchange for a free replacement battery. The monitor can continue to be used on AC power with power cord.</p>

<p><strong>Hazard: </strong>The stroller’s hinge mechanism poses a fingertip amputation and laceration hazard to the child when the consumer is unfolding/opening the stroller.</p>
<p><strong>Incidents/Injuries: </strong>The firm has received 15 reports of children placing their finger in the stroller’s hinge mechanism, resulting in 12 reports of fingertip amputations in the United States.</p>
<p><strong>Description: </strong>This recall involves all Maclaren single and double umbrella strollers. The word “Maclaren” is printed on the stroller. The affected models included Volo, Triumph, Quest Sport, Quest Mod, Techno XT, TechnoXLR, Twin Triumph, Twin Techno and Easy Traveller.</p>
<p><strong>Sold at: </strong>Babies“R”Us, Target and other juvenile product and mass merchandise retailers nationwide from 1999 through November 2009 for between $100 and $360.</p>
