<p><strong>Description: </strong>This recall involves child-sized frog-themed animal masks. The plush mask is green with yellow and red highlights. There are two eye cutouts and a green elastic band with a fastener used to secure the mask at the back of the child’s head. UPC code 06626491474 is printed on a label attached to the mask.</p>
<p><strong>Sold exclusively at: </strong>Target stores nationwide from August 2011 through September 2011 for about $1.</p>

<p><strong>Hazard: </strong>The drop-side rails on the crib can malfunction, detach or unexpectedly fall down, causing part of the drop side to fall out of position. When this happens, a space is created into which an infant or toddler can roll and become wedged or entrapped, which can lead to strangulation or suffocation. A child can also fall out of the crib. Drop-side incidents can also occur due to incorrect assembly and due to age-related wear and tear.</p>
<p><strong>Incidents/Injuries: </strong>CPSC and the firms are aware of nine incidents involving drop side rails that malfunctioned or detached, including one report of a child who sustained minor abrasions to the arm.</p>
<p><strong>Description: </strong>This recall involves Yu Wei full-size cribs sold under the Scroll and Lauren model names. The cribs were sold in antique white, pecan and dark cherry, and have the following model numbers and date codes listed on the inside of the crib’s end panels.</p>

<p><strong>Hazard: </strong>The drop-side rail hardware on the cribs can break or fail, allowing the drop side to detach from the crib. When the drop side detaches, a hazardous gap is created between the drop-side rail and the crib mattress in which infants and toddlers can become wedged or entrapped, posing risks of suffocation and strangulation. In addition, children can fall out of the crib when the drop-side rail falls unexpectedly or detaches from the crib. Drop-side rail failures can also occur due to incorrect assembly or with age-related wear and tear.</p>
<p><strong>Incidents/Injuries: </strong>CPSC and the firm are aware of 21 incidents involving drop sides that failed or detached. No injuries have been reported.</p>
<p><strong>Description: </strong>This recall involves wooden drop-side cribs with hidden drop-side hardware. The cribs were sold in various colors. Model numbers 202647, 211047 and 272547 are included in this recall. The cribs were sold separately and as part of the “City Lights,” “Fairy Tales” and “Dormez Vous” furniture collections. “Shermag” is printed on a tag on the mattress springs. The model numbers can be found on stickers and warning labels on the crib’s headboard or footboard.</p>
<p><strong>Sold at: </strong>The Land of Nod and other baby specialty stores from September 2004 through December 2008 for between $400 and $800.</p>

An 8-month-old girl sustained a minor injury on her head from exposed plastic hardware inside the portable crib.</p>
<p><strong>Description: </strong>The cribs are drop-side wooden cribs, painted or stained in black, cherry, dark brown, natural, white, and pink.</p>

<p><strong>Incidents/Injuries: </strong>CPSC and Arm&#8217;s Reach have received 10 reports of infants falling from the raised mattress into the bottom of the sleeper or becoming entrapped between the edge of the mattress and the side of the bed-side sleeper. No injuries have been reported.</p>
<p><strong>Description: </strong>This recall involves a product called a &#8220;co-sleeper&#8221; by the manufacturer. One side of the bed-side sleepers is lower than the others to allow positioning near a bed and access to the infant for care and feeding. This recall includes all Arm&#8217;s Reach Original and Universal styles with manufacture dates between September 1997 and December 2001. The manufacture date and model number can be found on a sticker on one of the product&#8217;s legs. Model numbers included in the recall begin with:</p>

<p>WASHINGTON, D.C. &#8211; The U.S. Consumer Product Safety Commission (CPSC) and Delta Enterprise Corp., of New York, N.Y. are re-announcing the 2008 <a href="http://www.cpsc.gov/cpscpub/prerel/prhtml09/09017.html">recall</a> of more than 985,000 drop-side cribs with &#8220;Crib Trigger Lock and Safety Peg&#8221; hardware. In January 2011, CPSC and Delta learned of a 2009 death in which 7-month-old girl from Colorado Springs, Colo. became entrapped and suffocated between the detached drop-side and mattress of her recalled crib. The crib was purchased secondhand and re-assembled without safety pegs in the bottom tracks.</p>
<p>Missing safety pegs can create a situation where the crib&#8217;s drop-side rail disengages from the track. This can create a hazardous space in which an infant can become entrapped and suffocate.</p>
<p>At the time of the October 2008 recall, CPSC notified consumers about the death of an 8-month-old girl who became entrapped and suffocated when the drop side of the crib detached. The crib involved in this incident also was re-assembled without safety pegs. At the time of the October 2008 recall announcement, there were reports of two entrapments and nine detachments in cribs without safety pegs.</p>
<p>&#8220;Buying or accepting cribs second hand can be risky,&#8221; said CPSC Chairman Inez Tenenbaum. &#8220;Second hand cribs may not come with all of the necessary parts that are needed to make sure your baby is safe. We urge parents and caregivers to use caution and to be aware that new rules established by CPSC will bring safer cribs to the market this summer.&#8221;</p>
<p>This re-announcement involves cribs that were made in Taiwan and Indonesia. The cribs were sold at major retail stores including Kmart, Target and Walmart between January 1995 and December 2005 (through September 2007 for model 4624) for about $100.</p>
<p>Delta&#8217;s name and address is printed on the mattress support boards and the Delta logo is on the crib&#8217;s top teether rail. Model numbers are located on the top of the mattress support board. This announcement includes the following 49 crib models with &#8220;Crib Trigger Lock with Safety Peg&#8221; drop-side hardware:</p>

<p><strong>Important Message from CPSC: </strong>CPSC reminds parents not to use any crib with missing, broken or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib, parents should check to make sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for parts separating that can create a gap and entrap a child. In addition, do not try to repair any side of the crib. <p><strong>Sold exclusively at: </strong>IKEA stores nationwide from October 2005 through June 2010 for about $80.</p>

<p><strong>Remedy: </strong>Consumers should stop using the crib immediately and check the crib. If the mattress support bolts extend through the nut, the bolts are the proper length and the crib is not included in the recall. If the bolt does not extend through the nut, the crib is included in the recall. Contact IKEA for a free repair kit for recalled cribs. In the meantime, find an alternate, safe sleep environment for the child, such as a bassinet, play yard or toddler bed depending on the child&#8217;s age.</p>

<p>The U.S. Consumer Product Safety Commission (CPSC) is warning consumers that &#8220;SafetyCraft&#8221; brand full-size and portable drop-side cribs manufactured or distributed by Generation 2 Worldwide of Dothan, Alabama, contain drop-side hardware that appears to be identical to Generation 2 Worldwide ChildESIGNS brand drop-side cribs recalled in <a href="http://www.cpsc.gov/cpscpub/prerel/prhtml10/10134.html">February 2010</a>. This hardware found on SafetyCraft drop-side cribs can fail and place infants and toddlers at risk of strangulation and suffocation. CPSC staff urges parents and caregivers to stop using these cribs immediately and find an alternative, safe sleeping environment for their baby. <strong>Do not attempt to fix these cribs</strong>.</p>
<p>Generation 2 Worldwide ceased operations in 2005. The &#8220;SafetyCraft&#8221; trademark was then purchased by Foundations Children&#8217;s Products of Medina, OH. <strong>SafetyCraft cribs manufactured and sold by Foundations Children&#8217;s Products are not the subject of this Safety Alert</strong>. Consumers can identify the Foundations SafetyCraft crib with &#8220;Foundations&#8221; printed on the mattress support assembly instructions label under the crib mattress.</p>
<p>Because Generation 2 Worldwide ceased operations, CPSC has limited information about these cribs and where they were sold. CPSC has been unable to identify major retailers that sold these cribs. The name &#8220;SafetyCraft&#8221; appears on a label affixed to the crib&#8217;s headboard or footboard. Product labels may identify the place of manufacture as Indonesia or Dothan, Alabama. This warning involves all SafetyCraft drop-side cribs, including model 92-8112, manufactured and/or sold by Generation 2Worldwide. Some of the cribs have a clear plastic headboard and footboard. These cribs are in use in child care facilities as well as homes.</p>
<p>The crib&#8217;s plastic drop-side hardware can break, which can cause the drop side of the crib to detach from a corner of the crib. When the drop side detaches, it creates a space into which an infant or toddler can roll and become wedged or entrapped. When a child is entrapped between the drop side and the crib mattress, it creates a risk of suffocation or strangulation. In addition, the crib&#8217;s mattress support can detach from the crib frame, creating a hazardous space in which an infant or toddler can become entrapped and suffocate or strangle.</p>
<p>CPSC received reports of three infant suffocation deaths and 20 drop side related incidents involving previously recalled models of Generation 2 Worldwide and ChildESIGNS cribs with plastic hardware similar to &#8220;SafetyCraft&#8221; cribs.</p>

<p><strong>Hazard: </strong>The drop-side rail hardware on the cribs can break or fail, allowing the drop side to detach from the crib. When the drop side detaches, a hazardous gap is created between the drop-side rail and the crib mattress in which infants and toddlers can become wedged or entrapped, posing risks of suffocation and strangulation. In addition, children can fall out of the crib when the drop-side rail falls unexpectedly or detaches from the crib. Drop-side rail failures can also occur due to incorrect assembly or with age-related wear and tear.</p>
<p><strong>Incidents/Injuries: </strong>CPSC and The Land of Nod have received 13 reports involving drop-side hardware that has malfunctioned or failed. No injuries have been reported.</p>
<p><strong>Description: </strong>This recall includes “Rosebud” cribs manufactured by Status Furniture. The cribs are white or antique white and have plastic drop-side hardware. “Status Furniture” appears on crib labeling on the lower portion of the headboard. Model number “910” appears on the assembly instructions.</p>
<p><strong>Sold at: </strong>The Land of Nod retail stores and online at www.landofnod.com from January 2003 through September 2004 for about $600.</p>
<p><strong>Manufactured in: </strong>Canada</p>
<p><strong>Remedy: </strong>Consumers should stop using these cribs immediately and contact The Land of Nod to receive instructions on how to receive a merchandise credit for the full purchase price of the crib ($599). The Land of Nod is undertaking this recall for its customers because Status Furniture is out of business. In the meantime, parents are urged to find an alternate, safe sleeping environment for the child, such as a bassinet, play yard or toddler bed depending on the child’s age.</p>

<p><strong>Hazard: </strong>The cribs&#8217; drop-side rail can malfunction, detach or otherwise fail, causing part of the drop side to detach from the crib. When a drop-side rail partially detaches, it creates a space between the drop-side and the crib mattress. An infant or toddler&#8217;s body can become entrapped in the space, which can lead to strangulation and/or suffocation. A child also can fall out of the crib. Drop-side incidents also can occur due to incorrect assembly and age-related wear and tear.</p>
<p><strong>Incidents/Injuries: </strong>The firm and CPSC are aware of 18 reported incidents in which drop-sides malfunctioned or detached from the crib. No injuries were reported. In one of the incidents a child became entrapped between the mattress and the drop-side. In three of the incidents children fell out of the cribs. The firm also has received 154 reports of drop-side hardware breaking during shipment, assembly or use.</p>
<p><strong>Description: </strong>The recalled cribs are wood with a metal mattress support and have a drop side with external plastic hardware. The cribs were sold in a variety of finishes. A label is attached to the footboard or headboard with the names &#8220;Bassettbaby&#8221; or &#8220;Bassett Furniture Industries, Inc.&#8221; as well as the model number, production date and other information. On some older models the manufacturer&#8217;s name appears on a separate label.</p>
<p><strong>Note: </strong>This recall does not include non drop-side cribs.</p>
<p><strong>Sold at: </strong>Children&#8217;s product stores, and other retailers nationwide and on-line from January 2000 through August 2010 for between about $200 and $500.</p>
<p><strong>Manufactured in: </strong>China, Croatia, Slovenia, Taiwan and Vietnam</p>
